There are numerous challenges pertaining to epilepsy care across Ontario, including Epilepsy Monitoring Unit (EMU) bed pressures, surgical access and community supports. We sampled the current clinical, community and operational state of Ontario epilepsy centres and community epilepsy agencies post COVID-19 pandemic. A 44-item survey was distributed to all 11 district and regional adult and paediatric Ontario epilepsy centres. Qualitative responses were collected from community epilepsy agencies. Results revealed ongoing gaps in epilepsy care across Ontario, with EMU bed pressures and labour shortages being limiting factors. A clinical network advising the Ontario Ministry of Health will improve access to epilepsy care.

Summary
Introduction
Theory, experiment and practice suggest that, when bidder valuations for multiple objects are super-additive, combinatorial auctions are needed to increase efficiency, seller revenue, and bidder willingness to participate (Bykowsky et al. 2000, Rassenti et al. 1982, Ledyard et al. 2002). A combinatorial auction is an auction in which bidders are allowed to express bids in terms of packages of objects. The now famous FCC spectrum auctions are a good example of the relevance of these issues. In 41 auction events from 1994 to 2003, the FCC used what is known as a Simultaneous Multiple Round (SMR) auction to allocate spectrum and raise over $40 billion in revenue. This auction format does not allow package bidding. The FCC auctions also divide the spectrum by geographic location. It is reasonable to expect that some bidders might receive extra benefits by obtaining larger, more contiguous portions of the spectrum. A firm might enjoy cost savings if they could purchase two adjacent locations. However, without package bidding, a bidder cannot express that preference, potentially lowering the efficiency and revenue of the auction. If the bidder attempts to acquire both licenses through bidding on the licenses individually, they might be forced to expose themselves to potential losses. The high number of bidder defaults on payments might, in part, be evidence of losses caused by the lack of package bidding. In response to these difficulties, the FCC plans to allow package bidding in future auctions (Federal Communications Commission 2002, Dunford et al. 2001). In particular, the FCC in its auction #31 for the upper 700 MHz band, affords bidders the ability to submit bids for packages of licenses. The particular design presented in this paper was developed prior to the FCC package auction design. Indeed one of the major features of the FCC design was clearly influenced by the pricing rules we developed herein.

OBJECTIVE
To measure transmission frequencies and risk factors for household acquisition of community-associated and healthcare-associated (HA-) methicillin-resistant Staphylococcus aureus (MRSA).
DESIGNProspective cohort study from October 4, 2008, through December 3, 2012.
SETTINGSeven acute care hospitals in or near Toronto, Canada.
PARTICIPANTSTotal of 99 MRSA-colonized or MRSA-infected case patients and 183 household contacts.
METHODSBaseline interviews were conducted, and surveillance cultures were collected monthly for 3 months from household members, pets, and 8 prespecified high-use environmental locations. Isolates underwent pulsed-field gel electrophoresis and staphylococcal cassette chromosome mec typing.
RESULTSOverall, of 183 household contacts 89 (49%) were MRSA colonized, with 56 (31%) detected at baseline. MRSA transmission from index case to contacts negative at baseline occurred in 27 (40%) of 68 followed-up households. Strains were identical within households. The transmission risk for HA-MRSA was 39% compared with 40% (P=.95) for community-associated MRSA. HA-MRSA index cases were more likely to be older and not practice infection control measures (P=.002–.03). Household acquisition risk factors included requiring assistance and sharing bath towels (P=.001–.03). Environmental contamination was identified in 78 (79%) of 99 households and was more common in HA-MRSA households.
CONCLUSIONHousehold transmission of community-associated and HA-MRSA strains was common and the difference in transmission risk was not statistically significant.

‘Choice’: what we mean by it, and what that means for preventing childhood obesity

Objective
(i) To map how US adults value ‘choice’ in the context of obesity policy and (ii) to discuss implications for obesity prevention in children.
DesignSemi-structured interviews (n 105) were conducted between 2006 and 2009 about causes of and solutions to childhood obesity. Quotes captured in field notes from community meetings (n 6) on childhood obesity prevention were also analysed. Each use of the word ‘choice’ and its variants was identified in these texts. Content and discourse were analysed to identify the implied values and meaning in each use.

SubjectsOne hundred and five adults, some involved in childhood obesity prevention initiatives.
ResultsThree distinct frames of ‘choice’ emerged: (i) having choices (choice as freedom), (ii) making choices (choice as responsibility) and (iii) influencing choices (contextual constraints and impacts on choice). Many speakers used more than one frame over the course of an interview. Most people using the third frame seemed to share the values behind the first two frames, but focused on conditions required to enable people to be accountable for their choices and to make truly free choices. A small subset thought outside the frame of individual choice, valuing, as one person put it, a ‘social contract’.
ConclusionsPublic debate in the USA about responsibility for and solutions to rising obesity rates often hinges on notions of ‘choice’. These frames, and the values underlying them, are not mutually exclusive. Respecting the values behind each ‘choice’ frame when crafting obesity prevention policy and employing all three in public communications about such policy may facilitate greater consensus on prevention measures.

Summary
This chapter discusses the outcome of in vitro fertilization (IVF) in medically complicated patients. A confounding factor for IVF pregnancies is the increased number of multiple pregnancies and the relatively increased number of high-risk pregnancies among women with chronic medical problems. Cancer patients present particular challenges to the IVF unit. Standard IVF protocols are used for controlled ovarian hyperstimulation in human immunodeficiency virus (HIV) discordant couples. The main concerns about IVF and malignant disease relate to the issue of the potential delay in the starting of the patient's chemotherapy or of any possible effect of hormonal changes on the cancer. Obesity might affect the outcome of IVF and pregnancy, but with careful management, a good outcome can be achieved. It has been suggested that systemic lupus erythematosus (SLE) may reduce the success of IVF-ET. The presence of antinuclear antibodies may reduce the implantation rate in IVF patients.

Aggleton & Brown have built a convincing case that hippocampus-related circuits may be involved in thalamic amnesia. It remains to be established, however, that their model represents a distinct neurological system, that the distinction between recall and familiarity captures the roles of these pathways in episodic memory, or that there are no other systems that contribute to the signs of amnesia associated with thalamic disease.
